Our physiotherapists and exercise physiologists provide high-quality, personalised care in your home, helping you recover safely, improve your mobility, and regain confidence in a familiar, everyday environment. A home visit allows your therapist to assess how you move around your home, understand the challenges you are experiencing, and create a treatment plan tailored to your needs and goals. Whether you are returning home after surgery, recovering from an injury, managing an ongoing condition or finding it difficult to attend the clinic, we can bring the support you need directly to you.

From hospital to home - and back to living well

The transition from hospital to home can feel challenging. Our home services helps bridge the gap between hospital discharge and returning to your usual activities. Starting rehabilitation early can help you move more safely, rebuild strength and feel more confident as you recover. Your therapist will work with you to develop a progressive rehabilitation program based on your procedure, current abilities and personal goals.

We provide pre- and post-operative rehabilitation for:

  • Hip, knee and shoulder joint replacements
  • Spinal surgery
  • Fracture fixation
  • Shoulder surgery
  • Arthroscopic procedures
  • Other orthopaedic surgery

When you are ready, your rehabilitation can continue in our Manly Vale clinic, where you will have access to private treatment rooms, a fully equipped rehabilitation gym, weights and strength-training equipment, and tailored exercise classes.

What can be included in a home physiotherapy or exercise physiology visit?

Every home visit is tailored to your individual needs and may include:

  • Pain management to support comfort and recovery.
  • Mobility, gait and balance assessment to improve safe movement, transfers, walking and confidence.
  • Falls prevention and home safety advice to reduce risk and improve independence.
  • Strength, conditioning and exercise rehabilitation to restore movement, strength and function.
  • Functional retraining and tailored home exercise programs to support everyday activities and ongoing progress.
  • Mobility aid and equipment advice, including safe use of crutches, walking frames and braces.
  • Education and support to help you manage your recovery and progress towards returning to work, recreation or sport.
  • Transition to clinic-based rehabilitation as your mobility and confidence improve.

Can I have physiotherapy or exercise phyiology home visits under the Support at Home program?

Yes, physiotherapy and exercise physiology services are included in the Restorative Care Pathway of the Support at Home program. This intensive, short-term period of care under the Support at Home program allows you to have 16 weeks of clinical care after an illness or injury to help you maintain or regain your independence.
 
The Restorative Care Pathway supports you to:
  • Retain your indepenence, delaying the need for higher levels of in-home care services
  • Regain your ability after illness or injury to carry out daily activities
  • Manage new or changing age-related conditions
  • Learn skills to better retain function as you age.

You may be eligible for up to two episodes of restorative care in a 12-month period. Talk to your GP or aged care provider to find out if you are eligible for physiotherapy or exercise physiology under the Restorative Care Pathway.

Rehabilitation that fits into your everyday life

One of the key benefits of home physiotherapy or exercise physiology is that we can assess you in the environment where you complete your everyday activities.

This helps us identify practical challenges that may not be apparent in the clinic. We can then tailor your exercises and recommendations around your home, daily routines and individual goals.

Our focus is not only on treating your immediate condition. We aim to help you move more confidently, regain independence and return to the activities that matter most to you.
